Any Japanese workers must join the public medical insurance system by Japanese law and it is also required that foreign workers are enrolled in the same scheme.
The public medical insurance system consists of two kinds of scheme that are the social insurance and the national health insurance.
The Social Insurance
If you are employed by the company registered in the social insurance system, you automatically become the insured person and the health card is provided by the company. You have almost nothing to do for the registration, and the insurance fee is withheld and paid by the company. And you need to remind that both the health insurance fee and the superannuation fee are withheld.
In the year of 2009, the premium rates of the social insurance are as followings;

The National Health Insurance
If you do your business by contract, not as employee, you must join the national health insurance. This registration must be done by yourself at the city office where your residence belongs to.
The calculation of premium rates is done based on your taxable income or local tax amount in the previous year (if you lived in Japan in that year). The concrete calculation is complex and depends on the local tax authority where you live. The rough maximum annual fee can be 720,000JPY.

*Any foreigner who lives in Japan more than 1 year must join the national health insurance, if not employed and joined the social insurance.
*From Apr.2010, it will be required to show his/her insurance card when applying the extension of Period of Stay.
